Admission Requirements & Enrollment Process
On or before your child/rens first day the following forms are required to begin at Creative Kidz Academy. A signed and completed application, feeding schedule (if applicable) a medical exam (signed by a physician) a current shot record, signed and dated, discipline and behavior management form, a signed documentation of receipt of summary of child care laws. After all paperwork is completed and signed by an administrator the date of enrollment will be set.
After the enrollment process is completed Creative Kidz Academy requires at least one hour of observation in the classroom by the parent to complete the enrollment process. This observation is required to ensure that both the parent and child are comfortable with the facility. This time period is also used to explain the class structure.
